Rates and Insurance
Dr. Melissa Jackson-Westbrook does psychological testing with individuals 6+ years of age.
Insurance Information
Dr. Melissa is in-network with:
Aetna, including Duke Select and Basic plans, and some Aetna CVS plans
Blue Cross Blue Shield (BCBS) (but does not accept Blue Home)
Cigna/Evernorth
Dr. Melissa can do a brief check of insurance benefits. However, she highly recommends calling your insurance company directly, before scheduling the intake, to see what their coverage is for psychological testing and/or therapy.
Some questions to consider when verifying insurance coverage:
1. What is my coverage for mental health services for therapy or psychological assessment (see insurance codes below)? Is Dr. Melissa in or out of network?
2. Do I have a deductible that has to be met before insurance covers a portion or all of my mental health services? If so, when does that deductible re-start?
3. If I don’t have a deductible, what is my co-pay or coinsurance? Coinsurance is a specific percentage of the cost of service that you will pay (i.e. 20% of a therapy session). A copay is a defined amount you owe per session (i.e. $25 for every therapy session).
4. Do I need a preauthorization/precertification for psychological testing?
*Always keep track of any calls/write down any reference numbers given to you when speaking to an insurance company representative.
Billing/CPT codes are below :
Dr. Melissa typically only does 60-min therapy sessions. The CPT code is 90837.
The CPT codes used to bill testing are:
• 90791 (Initial evaluation/intake for therapy or testing)
• 96130/96131 (Psychological evaluation services such as report writing, consultations, review of documentation, and interpretation of results)
• 96136/96137 (Test Administration and Scoring)
Additional Fees/Private Pay Rates:
No Show/ Late Cancellation Fee: $100 per hour (sometimes testing appointments are multiple hours)
Note: Each client will be required to sign a late cancellation/no show policy form before services begin. Dr. Melissa understands that emergencies will happen and will review each situation carefully (this may include illness and weather-related events).
Attendance at outside meetings (such as an IEP meeting): $300 per hour
Completed and signed forms for accommodations requests for school or work: $50
Completed disability packets or paperwork: $100
Private Pay Rate for Therapy: $200 for 60-min session
Private Pay Rate for Psychological Testing: $200 per hour (typically takes 8 to 12 hours for a full evaluation)
Private Pay Rate for Initial Intake for Testing or Therapy: $200
